Aesthetic Dentistry (Smile makeovers)

Smile makeovers are very personal, and vary from person to person. A smile makeover involves one or more cosmetic, dental or facial rejuvenation procedures to improve the aesthetics of your smile. Some of the process involve include Whitening, Veneers, Zirconia Crowns, Correction of crooked teeth and Diastema.

Below are some ways to improve your smile:


  • Brush your teeth twice a day, and use a mouth wash to rinse your mouth at least once a day.

  • Visit your dentist at least annually to check for cavities before they become bigger and more complex.

  • Repair and replace your worn restorations before they create new problems.

  • Minimize food with high sugar content can reduce acid production in the mouth.

  • Avoid metal fillings such as amalgam (silver color) fillings that have mercury. White color fillings are equally long lasting and also aesthetic.

  • If your teeth are not straight, you can look for an orthodontist  to fix them.

  • If your teeth are dark, you can get whitening product at your dentist to apply on the surface. For a stronger effect, your dentist can whiten for you in a one-hour session, with a home kit for your maintenance.

  • For sensitive teeth, you can do Veneers, facings that cover up the teeth surface.

  • If you don’t like Veneers, you can do Porcelain crowns or Empress crowns to cover the entire teeth. But in the process, there will be a lot of trimming on your natural teeth surface.

  • If you have missing teeth, try to replace them by a denture, bridge or implants. You need to balance the cost, comfort and how long the treatment can last.

  • If you have bad breadth, you need a scaling and polishing done together with mouth rinse. Regular exercise help your thyroid function and metabolism which are great for your general well being and toxic excretion.

Crowns

Crown refers to the restoration of teeth using materials that are fabricated by indirect methods which are cemented into place. It is done by placing a tooth-shape “cap” over the tooth, encasing the entire visible portion of the tooth that lies above the gum line. When cemented, it will help to restore the shape, size, strength and appearance of the teeth.

Bridges

Bridges use teeth (on either side of a gap) to support the replacement teeth. The supporting teeth are like piers on the sides of the river banks supporting the bridge.
There are four main forms of bridges:

Fixed-Fixed Bridge
Crown preparations are made on either side of the gap. A metal framework with three porcelain teeth, carefully color-matched and shaped, are bonded, the center tooth being a false one. The two supporting crowns join with the false tooth, and the whole structure is cemented on the teeth.

Fixed-Movable Bridge
The bridge has a fixed end with a movable hinge at the other end. This normally used for patients with difficult bites.

Maryland Bridge
The bridge is resin bonded with the back of the adjacent teeth etched for the metal wings. Strong composite resin cement bonds the metal wings to the teeth. The metal wing cannot be seen from the front.

Cantilever Fixed Bridge
An anchoring tooth is crowned and attached to one or few pontics (false tooth). This is a more conservative method, but not as long lasting as the conventional fixed-fixed bridge.

Correction of Crooked Teeth

There are a few treatments available for crooked teeth depending on the severity. Orthodontics (braces) is the most preferred way for sever cases, and simple Bonding for mild cases. If the patients do not want to wear braces, they can go for crowns or bridges.

Root Canal Treatment

Root canal therapy is a complex procedure that requires both skill and experience. The dentist numbs the area of the infected tooth with local anesthesia (usually xylocaine or bupivacaine). A clamp is placed over the tooth, and a rubber membrane (rubber dam) is spread over the clamp to isolate the tooth and prepare it for the operation.

A small hole is made through the enamel and dentin, and into the pulp. The pulp is then removed with small stainless steel files of increasing diameter. After the pulp has been removed, the inside walls of the roots are shaped, almost like a sculpture. Nickel/Titanium files that fit on a slow speed drill can aid in shaping the canals. During the procedure, fluids (irrigants) such as sodium hypochloride (bleach) and a compound containing ethylenediamine-tetraacetate (EDTA) are used to both kill remaining pulp tissue and bacteria within the roots, and remove dentin shavings produced by the files.

After the pulp is removed and the inside of the roots shaped, the canal is filled by sterile, thin column of compacted paper point which are needle shaped made of sterile paper which can be further impregnated with antibiotics or antiseptic solutions, and a temporary filling.

Usually the Root Canal Treatment requires 3 visits. During the second visit, the canal is irrigated again to ensure a bacteria free canal. Calcium hydroxide is then placed in the canal with temporary filling again. The third visit, the dental surgeon closes the canal with Gutta Percha. Gutta-percha is a miraculous substance that was first introduced by Bowman in 1867. It is a purified form of Mazer Wood Trees indigenous to Indonesia and Malaysia that is combined with zinc oxide and other materials to form the rubbery filling that is placed into the tooth roots. The Gutta-percha is then cemented into the roots with a sealer that usually contains zinc oxide and eugenol. The goal of the filling procedure is to hermetically seal off the tooth against bacteria.

There are two main techniques to filling a root canal: lateral condensation and warm vertical condensation. Although no conclusion has been drawn by any research, warm vertical condensation appears to have the advantage of more completely filling the tooth roots, especially the accessory canals.

Orthodontics (Braces)

Orthodontics/braces are used for teeth strengthening and is the best way to correct malpositioned or crooked teeth. Forces are applied to individual teeth to put them in proper position in the jaw bones, which can often alter facial bones of very young patients. The treatment takes 6-30 months, usually requiring few weeks of getting used to. After treatment retainers are required few nights weekly.

Orthodontics

There are 3 types of orthodontic appliances:

Functional Appliances
These removable appliances are designed to improve or release the growth potential of the jaws. Screw appliances are used to expand the plate to widen the roof of the mouth (Palate). Other appliances are also used to counteract muscular forces that stop the lower jaw from growing. Theses appliances are for patients starting treatment at a very young age and for skeletal discrepancies.

Fixed Appliances
These appliances are bonded to the teeth. Wires apply pressure to individual teeth, moving them into position. Typical treatment lasts 21 months, followed by wearing retainers (Hawley’s Retainers) for at least 6 months to a year to stabilize. This depends on the duration of the worn braces in regards to the treatment plan (Type of occlusion and skeletal jaw relationships).

Removable Appliances
These are normally plastic plates with wires and springs or screws that apply pressure to the selected teeth. These types of appliances is usually used to maintain changes relationships rather than changing the relationship.

Implants

Implants must be performed by implant specialist. These are performed when there is a good amount of ridge available for the implant insertion,with the patient in relative good health.

There are two stages:

Stage 1
The implant is inserted and is successful if there is no infection and the osteo-integration, where the implant and bone are healed tightly together, are well established. The implant is biocompatible with the patient ridge.

Stage 2
The crown will be added. Review and proper hygiene is compulsory for the patient , as well as regular 6-12 months scaling by your dentist.

Whitening (BriteSmile)

There are two different methods of whitening. The first method is the Laser (Britesmile) which the teeth can be whitened within one hour. The second method is Bleaching which will be further discussed in the following paragraph.

Bleaching uses a strong oxidizing agent (Hydrogen Peroxidel) to lighten the teeth up to 2 to 3 degrees. During the treatment, there may be a temporary slight discomfort caused by the oxidizing agent. The process is similar to hair bleaching. As bleaching is not recommended for thin hair, similarly, it is not recommended for thin enamel (covering tooth surface), which may cause sensitivity, hindering the treatment.

Composite Restorations (Tooth Color Fillings)

Composite is a mercury free tooth colored material composed of glass particulates suspended in a resin matrix. This relatively inexpensive component is widely used for the restoration of dental cavities and the replacement of the dental amalgam (metal alloys) fillings which contain high amount of mercury (which can be swallowed or inhaled) and the restoration of worn out or chipped teeth. Composite restorations are often used for aesthetic reasons to enhance the contour, shape, color or relative position of the front teeth.

Root Planning

Root planning involves the cleaning away of the plaque, calculus, tar from the root surface (cementum).

The treatment of the loose tooth depends on the cause. For infected nerve, root canal treatment and regular deplaquing can resolve the problem. For perio-endolesion case, the bacteria have traveled from the gum to the periodontal fibers, causing the widening of the lamina dura, leading to nerve damage, and subsequently resulting in loose tooth. This can be treated with a root canal treatment followed by proper scaling of the gum pocket. If the looseness is due to gum related problem but does not involve the nerve, proper scaling and root planning  is the primary treatment, followed by splinting and interconnected crowns, depending on the severity of the looseness.

Prosthodontics (Full/ Partial Denture)

Dentures are prosthetic devices constructed to replace missing teeth, and which are supported by surrounding soft and hard tissues of the oral cavity. Conveentional dentures are removable, but there are many other different designs, some of which rely on bonding or clipping onto teeth or dental implants. There are mainly two type of dentures: Complete & Partial Dentures.

Complete Denture
Complete dentures cover your entire jaws, either upper or lower, resting directly on the gum that covers the bone. One or more natural teeth may be kept when a denture is made. These teeth usually have root canal treatment and are shortened to fit under the denture. This type of denture is known as overdenture. Overdentures can also fit over implants., or a metal bar can link implants together , proving support for the denture.

Partial Denture
Removable partial dentures are made up of a metal framework with plastic teeth and gum areas. The framework include metal clasps or other attachments to hold the denture in place. Fixed partial dentures, or bridges, are cemented in place and better simulate natural teeth. There are different types of bridges, depending on how they are fabricated and the way they anchor to the adjacent teeth. Conventionally, bridges are made using the indirect method of restoration however, bridges can be fabricated directly in the mouth using such materials as composite resin. There are special types of partial dentures such as Nesbit Denture which replaces one or more back teeth and Flipper Denture which is made of acrylic and replaces one or more teeth temporary until another form of treatment like bridge or implant is decided.
